Rates and Insurance

Please check with your insurance company regarding out-of-network benefits and pre-authorization requirements that might exist.
We are a Medicare provider, and accept assignment for services provided to individuals 65 years and old, and who have Medicare insurance. We do not accept any other form of insurance.
We also provide independent neuropsychological evaluations for students in public schools in New Jersey and neighboring states. If your child's school district has agreed to pay for an independent neuropsychological evaluation, we will set up payment arrangements directly with the district.
Payment for Services
We will help with the pre-authorization and reimbursement process, but ultimately the client is responsible for payment of fees at the time of service. We will provide you with a statement that contains all the information you need to submit for reimbursement from your insurer. Insurance reimbursement varies across carriers and also by the type of service to be reimbursed. We do not guarantee that you will receive reimbursement from your insurance carrier.
Cash, check, Visa, and MasterCard are accepted for payment.
Reimbursement for Neuropsychological Evaluations
In order to increase the likelihood of obtaining insurance reimbursement for the neuropsychological evaluation, a letter of medical necessity from a physician stating why the treatment or evaluation is needed will be helpful. Then, the client should submit the letter along with the appropriate CPT code to the insurance company for pre-authorization of benefits. Having a neurological basis for the evaluation, such as brain injury, suspected genetic disorder/auditory or visual processing disorder, or change in cognitive functioning, is often necessary for reimbursement.
The CPT code for a neuropsychological evaluation is 96118. If the client pursues a pre-authorization of benefits for a neuropsychological evaluation, use only the term “neuropsychological evaluation”. Medical necessity is usually very important in getting reimbursed, but the amount that is reimbursed is not assured.
Cancellation Policy
If you do not show up for your scheduled therapy appointment, and you have not notified us at least 24 hours in advance, you will be required to pay the full cost of the session.
Neuropsychological Evaluations must be cancelled one week in advance of the first scheduled session.
